Carelon Case Management

Case Management is a proactive healthcare strategy that identifies people with high use of emergency room and hospital services. This provides targeted interventions to improve overall health outcomes. Once a participant gets a case manager, they will review their specific needs and create a personalized care plan. Case managers work with children, adolescents, and adult participants to connect them with essential community support services and resources.

Maryland’s Coordination of Care

Participants receiving substance use disorder residential treatment (levels 3.3, 3.5 and 3.7) supported by Behavioral Health Administration funding are eligible to receive Maryland State Care Coordination as well as targeted, high-risk populations, such as: 

  • Pregnant women 
  • Women with children 
  • HIV positive individuals 
  • IV drug users 
  • Court-ordered participants 

Maryland State Care Coordinators are based in each jurisdiction at the county health departments. Care Coordinators receive a referral from the residential program while the patient is still in treatment.  

Care Coordinators assist patients as they transition into the recovery community by providing a connection to community/faith-based services and other human services organizations.
